Hospice @ Home
The Hospice now has a Hospice at Home service, the purpose of which is to provide care for patients whose wish is to remain at home.
Co-ordinated by an experienced Specialist Palliative Care Nurse, this service enables patients to be cared for at home during the final stages of their life, if that is their wish, and has expanded. It is now able to offer care in the patients home for up to a month, usually from 8am to 9pm, but a 24 hour rota basis can be arranged if needed.
Referrals to the Hospice at Home service are made via GP’s, District Nurses or Hospitals.
