This new sharing agreement allows clinicians at St Raphael’s Hospice to view a patient’s GP record and vice versa.

Jenny adds:

‘Our patients’ complex needs are looked after by many different professionals in the inpatient and community setting, teams include our own specialist palliative care service, GPs, hospital specialists and dedicated community nursing and allied healthcare professionals. This new sharing agreement effectively allows us to ‘electronically talk to each other’, improving the speed and accuracy of information passed between services, avoids unnecessary repetition and allows us to spend more time delivering care to those who need us most.’
